YOUR HEALTH INFORMATION
We are committed to protecting your Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
HOW WE USE AND DISCLOSE YOUR INFORMATION
We may use and share your PHI without your written authorization for:
Treatment
- Dispensing prescriptions and compounded medications
- Communicating with your prescriber
- Providing counseling and care coordination
Payment
- Processing payments (e.g. credit/HSA cards)
- Billing when applicable
Healthcare Operations
- Quality assurance and compliance
- Licensing, audits, and regulatory requirements
- Staff training and pharmacy operations
MAIL-ORDER & COMMUNICATIONS
As a mail-order pharmacy, we may:
- Ship medications to your address using third-party carriers
- Include necessary information to ensure accurate delivery
We take reasonable steps to limit PHI on packaging. We may contact you by phone, voicemail, text, or email regarding your prescriptions. You may request alternative communication methods at any time.
OTHER USES
We will obtain your written authorization before using or sharing your PHI for purposes not described above, including marketing. You may revoke authorization at any time in writing.
YOUR RIGHTS
You have the right to:
- Access and obtain a copy of your records
- Request corrections to your information
- Request restrictions on certain uses/disclosures
- Request confidential communications
- Receive an accounting of disclosures
- File a complaint without retaliation
OUR RESPONSIBILITIES
We are required to:
- Maintain the privacy and security of your PHI
- Provide this Notice
- Notify you of a breach affecting your PHI
- Follow the terms of this Notice
COMPROMISED PACKAGE INFORMATION
- If the integrity of your package was compromised during shipping or if your medication requires certain storage conditions that were not met during shipment, please contact your pharmacy.
ADDITIONAL INFORMATION
- Written information about your prescription(s) has been provided for you. Please read all the information including package inserts and notice of privacy practices before you take your medication. If you have any questions, a pharmacist is available during normal business hours to answer these questions.
