Tretinoin (0.03%, 0.05%) | Hyaluronic Acid 0.5% | Niacinamide 3%

Tretinoin (0.03%, 0.05%) | Hyaluronic Acid 0.5% | Niacinamide 3% is a dermatology-focused preparation for prescriber-directed skin protocols. Ingredient selection should reflect the patient's diagnosis, skin type, tolerability, pregnancy status, and treatment goal.

CreamTopicalRx Only503A Compounded

This compounded topical preparation combines tretinoin with hyaluronic acid and niacinamide—a triple-ingredient approach to retinoid-based skin therapy. Tretinoin provides the pharmacologically active retinoid benefit (keratinocyte normalization, collagen induction, depigmentation). Hyaluronic acid serves as a humectant to maintain skin hydration. Niacinamide adds complementary depigmenting activity, anti-inflammatory effects, and barrier enhancement. Together, this combination maximizes tretinoin's clinical benefits while substantially improving tolerability. Genesis Compounding prepares this as a prescription-only, patient-specific 503A compounded preparation and it is not FDA-approved as a compounded product.

Active IngredientPharmacologic Role
Tretinoin (0.03% or 0.05%)Retinoic acid receptor agonist that normalizes follicular keratinization, accelerates epidermal renewal, induces procollagen, suppresses MMP, and reduces melanin for acne, photoaging, and hyperpigmentation treatment.
Hyaluronic Acid 0.5%Humectant glycosaminoglycan that retains water in the stratum corneum, counteracting tretinoin-induced dryness and improving skin hydration and texture.
Niacinamide 3%Vitamin B3 derivative that inhibits melanosome transfer, provides anti-inflammatory and barrier-reinforcing effects, and reduces the visible signs of retinoid irritation.

Applied topically once nightly to the face or affected area. Apply a small amount to clean, dry skin. Avoid eye contact and mucous membranes. Allow to dry before applying moisturizer if needed. Use daily SPF 30+ sunscreen each morning throughout treatment.

Apply a small amount (pea-sized) once nightly as directed by the prescriber. Concentration (0.03% vs 0.05%) is selected by the prescriber based on skin type, indication, and prior retinoid experience. Titration: initiate at lower concentration or alternate-night dosing if retinoid naïve, advancing to nightly use after 2–4 weeks.

  • Tretinoin: Activates RAR/RXR nuclear receptor heterodimers in keratinocytes, driving gene expression changes that increase epidermal cell turnover, stimulate collagen I/III synthesis, block inflammatory AP-1 transcription factor, suppress MMPs, and reduce tyrosinase activity and melanin accumulation.
  • Hyaluronic Acid: High-molecular-weight humectant that holds water molecules within the epidermis, maintaining the skin moisture gradient disrupted by tretinoin. Mitigates TEWL and desquamation.
  • Niacinamide: Inhibits the transfer of melanosomes from melanocytes to keratinocytes; upregulates barrier ceramide synthesis; exerts anti-inflammatory effects via inhibition of NF-κB and reduction of cytokine release, reducing post-inflammatory hyperpigmentation and retinoid-associated erythema.

This triple combination is used for acne, photoaging, melasma, PIH, and general skin quality improvement. The inclusion of both a hydrating (HA) and skin-barrier-reinforcing (niacinamide) agent makes this formulation particularly suited for patients with sensitive or dry skin who require retinoid therapy but have difficulty tolerating dryness and irritation.

Contraindications:

  • Hypersensitivity to any component
  • Pregnancy (first trimester especially)
  • Sunburned or acutely inflamed skin

Warnings & Precautions:

  • Photosensitivity: daily sunscreen mandatory
  • Retinoid dermatitis may occur initially; start with lower concentration or alternate-night use
  • Avoid concurrent harsh exfoliants or alcohol-based products

Common Side Effects: Erythema, peeling, dryness, and stinging, especially during initiation. Hyaluronic acid and niacinamide substantially reduce these effects.

Store at room temperature (15–25°C) in opaque, sealed packaging away from light. Do not freeze. Use within the Genesis Compounding-assigned beyond-use date.

What is the benefit of having all three ingredients in one preparation?

One application delivers the therapeutic action of tretinoin along with immediate hydration (from hyaluronic acid) and complementary depigmenting plus barrier-repair effects (from niacinamide), reducing the complexity of a multi-step skincare routine and improving tolerability.

Can this be used on sensitive skin?

Yes—the combination of HA and niacinamide makes this formulation substantially better tolerated than tretinoin alone, but prescribers should initiate at the lower (0.03%) concentration and alternate-night application for sensitive skin types.

Do I still need a separate moisturizer?

The hyaluronic acid provides humectant hydration; however, patients with very dry skin may still benefit from a bland occlusive moisturizer applied over the preparation.

Is this FDA-approved?

This is a 503A compounded preparation by Genesis Compounding. Each component has well-established dermatologic use; this specific combination is not FDA-approved as a compounded product.

When should I expect to see results?

Improvements in acne may be visible at 6–8 weeks; photoaging and pigmentation changes typically require 12–24 weeks of consistent nightly application.
