Tretinoin 0.045% | Hydroquinone 6% | Hyaluronic Acid 0.5%

Tretinoin 0.045% | Hydroquinone 6% | Hyaluronic Acid 0.5% is a dermatology-focused preparation for prescriber-directed skin protocols. Ingredient selection should reflect the patient's diagnosis, skin type, tolerability, pregnancy status, and treatment goal.

CreamTopicalRx Only503A Compounded

This compounded topical combination pairs the two-agent depigmenting standard (tretinoin + hydroquinone) with hyaluronic acid as a humectant to improve tolerability. Tretinoin accelerates epidermal turnover and melanin dispersal while enhancing hydroquinone penetration; hydroquinone 6% inhibits tyrosinase to reduce melanin synthesis; and hyaluronic acid 0.5% provides deep hydration to offset the dryness and irritation intrinsic to this regimen. Genesis Compounding prepares this as a prescription-only, patient-specific 503A compounded preparation and it is not FDA-approved as a compounded product.

Active IngredientPharmacologic Role
Tretinoin 0.045%Retinoic acid receptor agonist that accelerates keratinocyte turnover, disperses melanin, enhances hydroquinone penetration, and reduces post-treatment repigmentation.
Hydroquinone 6%Tyrosinase inhibitor that blocks melanin biosynthesis at the committed step (L-DOPA conversion), providing primary depigmenting activity.
Hyaluronic Acid 0.5%Glycosaminoglycan humectant that retains moisture in the stratum corneum, countering retinoid- and hydroquinone-induced dryness and improving patient adherence and skin comfort.

Applied topically once nightly to hyperpigmented areas on clean, dry skin. Apply a thin layer, avoiding eyes and mucous membranes. Use daily SPF 30+ broad-spectrum sunscreen each morning. Do not apply to sunburned, broken, or infected skin.

Apply a thin layer to affected areas once nightly. Treatment is prescriber-determined in duration, typically 3–6 months of continuous use for the hydroquinone component. Reassess response at 8–12 weeks.

  • Tretinoin (0.045%): RAR/RXR nuclear receptor activation → accelerated keratinocyte turnover, melanin dispersal, collagen induction, and enhanced ingredient co-penetration.
  • Hydroquinone (6%): Competitive tyrosinase substrate inhibitor blocking DOPA → dopaquinone conversion; direct melanocytotoxic effects at higher concentrations.
  • Hyaluronic Acid (0.5%): High-capacity water-binding glycosaminoglycan that replenishes the moisture content of the stratum corneum, maintaining the skin barrier integrity disrupted by retinoid and hydroquinone exposure.

Used for moderate-to-severe melasma and PIH where the dual depigmenting efficacy of tretinoin and 6% hydroquinone is needed but tolerability is a concern. Hyaluronic acid addition specifically addresses patient-reported concerns about dryness and irritation that limit adherence to standard hydroquinone/tretinoin regimens.

Contraindications:

  • Hypersensitivity to any component
  • Pregnancy (avoid first trimester)
  • Active skin infection or inflamed skin at application site

Warnings & Precautions:

  • Ochronosis: limit continuous hydroquinone use to 3–6 months
  • Photosensitivity: mandatory daily sunscreen
  • Retinoid dermatitis: erythema and peeling expected at initiation
  • Systemic absorption of hydroquinone: avoid large surface area application

Common Side Effects: Erythema, peeling, dryness, and burning. Hyaluronic acid significantly reduces dryness-related side effects.

Store at room temperature (15–25°C) in sealed, opaque packaging, protected from light and air. Hydroquinone oxidizes upon prolonged air and light exposure; discard if brown discoloration develops. Do not freeze. Use within the Genesis Compounding-assigned beyond-use date.

Why is hyaluronic acid included in this depigmenting formula?

Tretinoin and high-concentration hydroquinone together can cause significant skin dryness and irritation. Hyaluronic acid, a powerful water-binding molecule, is added to maintain skin hydration, reduce irritation, and improve the comfort and adherence of this regimen.

Can I use a separate moisturizer on top of this?

A bland, fragrance-free moisturizer may be applied after the preparation has absorbed if additional hydration is needed—ask your prescriber. Avoid products with active ingredients that might interact with tretinoin or hydroquinone.

How long will it take to see results?

Initial improvement in skin tone may be seen at 4–8 weeks; more significant results for melasma or deep PIH typically require 12–24 weeks of consistent nightly use with strict sun protection.

Is this FDA-approved?

This is a 503A compounded preparation by Genesis Compounding and is not FDA-approved as a compounded product.

What are the main safety concerns with this formula?

The primary concerns are ochronosis from prolonged hydroquinone use (limit to 3–6 months continuous), retinoid dermatitis, increased UV sensitivity, and systemic hydroquinone absorption with large surface area application.
